When you are admitted into a professional program, you may submit a single professional field and clinical application, which may be used for all the courses in your program that require field placements. (See a list of these courses for each professional program.)

Note: For non-cohort courses, you must still complete the application process each semester for service-learning or standard field experiences. Please contact your academic advisor if you are unsure if a course is part of your professional cohort.

Placement Notifications

  • Mid-May Placements for fall semester announced for field experiences (BLKS 2 and 3) and clinical practice and final internships.
  • Two weeks after Drop/Add Students receive placement confirmations for Block 1 field experiences.
  • Mid-December Placements for spring semester announced for field experiences (BLKS 2 and 3) and clinical practice and final internships.

How To Complete the Professional Field and Clinical Application

  1. Choose and complete the appropriate background check for your program (Pre-Service Certificate, PreCheck, or DECAL).
  2. Print, complete, and scan into a PDF the both the FERPA form and Professional Tort Liability Insurance form below. You will be expected to upload these into the application.
  3. Complete the online form. Note the entire application must be completed in one sitting.

Background Checks

Select the Right Background Check

  • Pre-Service Certificate Apply for this certificate if you are in a teacher-education program. You will be issued a MyPSC/Georgia CERT ID #, which will serve as proof of a clear background check.
  • Background Check (PreCheck) This background check is required if you are in a program other than teacher education, or are a teacher-education major who has not been officially admitted into your program. Note: PreCheck background check reports are considered valid for one year from date of purchase.
  • DECAL: Bright From the Start National Fingerprint Background Check This additional fingerprint check will be needed if you will be placed in an early learning center or prekindergarten program licensed by the Department of Early Care and Learning. You will receive notice from the Office of Student Services if this background check is required.


    Please make sure to print and upload both pages, the signature page and the Exhibit A page. This form must be renewed annually.

  • Tort Liability Insurance
    Submit this form only one time if insurance is waived. You must resubmit the form if tort liability insurance is purchased and has been renewed since your last submission. This form is not required by athletic training majors, who are covered under a group policy through the academic program. This form is only required for students in the College of Education. Non-COE students do not need to complete this form.

Note: For COE students, we highly recommend you purchase tort liability insurance. It can be purchased for a minimal cost through student membership in professional organizations such as GAE or PAGE.
