To nominate someone for a Distinguished Alumni Award, please submit:
- Letter of nomination, including award category
- Two letters of recommendation
- Nominee’s resume or CV
- Contact information for nominator and nominee, including mailing address, email address, and phone number

Lifetime Achievement Alumni Awards
